In order to chaperone a field trip or volunteer in the classroom or as a library parent, all parents must have an up-to-date CORI form on file.  If you have not filled out a CORI form since September 2015, your information is out dated and must be renewed this year.  Next time you are at the school to pick-up or drop-off your kids or for parent teacher conferences, please stop by the front office and ask for a CORI form to fill out.  The school also will need to make a copy of your driver’s license.

The CORI form is required by MA law and allows schools to screen volunteers for any Criminal Offender Record Information (CORI).
